Yes, the Weight Loss and Natural Hormone Balancing Clinic is independently owned and operated by Kathryn R. Palmer, FNP and Primary Care Provider (PCP). Areas of specialty interest include hormone balancing, nutrition and weight loss support. Kathryn’s goal is to help clients achieve optimal health in the most natural way possible.

2. How do I schedule an appointment?
To schedule an appointment with Kathryn, call (801) 272-1246 or send a message from our Contact page. Click below on either “Female – New Patient Paperwork” or “Male – New Patient Paperwork” to download and print. Fill out all forms completely before your scheduled appointment. If you have scheduled a face-to-face appointment, please bring the completed paperwork with you. 3. What are your office hours?
The clinic is open Monday, Wednesday and Friday from 9:00 AM to 5:00 PM MST, by appointment only.

4. What can I expect after scheduling an appointment for hormone balancing?
Because everyone is unique and has a different situation, no course of treatment will be exactly the same. However, there are certain things that you can expect:

5. Does your clinic accept cash, personal checks and credit cards?
Yes, we accept Visa, MasterCard, Discover Card, personal checks and cash. Additionally, PayPal is offered as a convenient method of payment for all our products and services.

6. As a patient, will I be provided with a receipt including diagnostic and procedure codes so I can submit my own claims to my insurance company for reimbursement?
Yes, currently our Nurse Practitioner is an out-of-network provider, and we no longer accept direct reimbursement from insurance companies. Payment for service is due at the time you schedule an appointment. Neither prior authorization nor claim submission services are offered. However, following your consult, you will be provided with a Receipt and Insurance Reimbursement Form that will include appropriate ICD-9 and CPT codes. Additionally, many insurance companies offer flex-spend accounts that cover products for weight loss and essential nutritional supplements.

7. What is RelayHealth? Is it FREE, and how do I sign up?
RelayHealth is a convenient way to communicate with our Front Desk. It’s a safe, secure way to communicate your non-urgent healthcare needs. It’s as easy to use as Email, but incorporates stronger security measures that ensure your medical privacy in accordance with the Health Insurance Portability and Accountability Act (HIPPA). It’s FREE to sign up. Your web posts to RelayHealth will be answered within eight (8) hours on the NEXT BUSINESS DAY. Clinic Business Days are MONDAY, WEDNESDAY and FRIDAY.

DO NOT use Relay Health for rescheduling or canceling appointments. To avoid a $50 late cancellation fee, please contact our Front Desk directly (801-272-1246) at least 24-48 hours in advance  to take care of a schedule change.
